Registration Facts
Each attendee must complete and return a registration form to IHA by September 9, 2011. Fees for luncheons and other functions are to be enclosed with the registration form, with checks made payable to IHA. If additional forms are needed, photocopies of the registration form are acceptable. Only one registrant per form; lists of names and addresses will not be accepted for registration. Name badges, tickets and onsite agendas may be picked up at the IHA registration desk located on the third floor of the Marriott during registration hours throughout the conference.

Meal Tickets
Meal costs are listed on the registration form. Ticket requests are processed in the order received. If available, unallocated tickets will be sold at the registration desk at an additional cost of $5.00 per ticket. Meals are guaranteed in advance; therefore meal tickets are not refundable after September 23.
Program Fees
The Annual Meeting is a benefit of IHA membership. Employees of IHA member hospitals and health systems; IHA Associate Members, IHA Annual Meeting Sponsors and members of IHA Affiliate Groups may attend the education sessions at no charge unless noted otherwise. Hospital administration graduate students, not employed by a hospital, may attend for $25. Individuals not affiliated with IHA member institutions may attend for $500. Solicitation of IHA members is strictly prohibited.
Cancellation
- Substitutions welcome anytime via fax or e-mail.
- Meal ticket refunds will be made if received prior to September 23, 2011.
- All cancellations and substitution requests must be sent to Crystal Peters (petersc@ihaonline.org) by e-mail or faxed to 515.698.5131 or 515.283.9366.
What to Wear
Business casual is the suggested attire. Please remember that meeting room temperatures are often difficult to moderate, so you may need a sweater or jacket.
Parking
A limited number of underground parking spaces are available for $17 per day for registered overnight guests of the Marriott. Alternative parking spaces are available at city parking ramps located at 8th and Locust (Locust Street Mall), 7th and Grand, 5th and Walnut, and 4th and Grand.
